Description
Shaeer’s Dorsal Phalloplasty (DP) is a length enhancement procedure that can be performed either stand-alone or concomitantly with penile prosthesis implantation (PPI). The principle is tacking-in the peno-pubic junction (PPJ) to the pubic periosteum with non-absorbable sutures, in candidate patients with a moderate pubic fat pad that conceals the penis.
We demonstrate a case with concomitant PPI and DP. The procedure is performed through a peno-scrotal incision, through which the penile prosthesis can be implanted in the same procedure. Following a peno-scrotal skin incision, and dissecting down to the tunica albuginea, the stay sutures for PPI are placed. The base of the penis is retracted sideways and blunt dissection is performed in the dorsal midline down to the pubic bone. A size 5 Polyster suture is passed through the pubic periosteum in the midline. The suture line is then passed though the dermis of the pubic skin. The point in the dermis is determined such that pulling it in will reveal the penis rather than pull the penis back. PPI is carried on as usual, and the polyester suture line is tied following implantation to tack the PPJ to the pubic bone, revealing the penis. This tacking suture can be a single on in the midline, or two more can be added from the pubic tubercle on either side to the PPJ.
