Shaeer’s Approach for Inflatable Penile Prosthesis Implantation: The Deeper Corporotomies

Description

Low corporotomies are key for minimizing the rear tip extenders (RTEs) of the inflatable penile prosthesis (iPP) thereby improving axial rigidity (1), as well as minimizing the length of tubing withing the crural bodies. However, contents of the scrotal sac are usually in the way, possibly requiring a wider incision, more operative time, and more retraction. Difficulty in proximal exposure is most pronounced when the corporotomies are to be sutured closed. Access to the crura is also important in cases with corporal scarring. Shaeer’s approach facilitates access to the crura and low placement of the corporotomies. The hip joint is flexed and externally rotated, and the knee joint is flexed. This allows the scrotum to hang low on the draped perineum, getting it out of the way and allowing easy access to the crura. The patient is placed in the reverse Trendlenberg position, allowing a better viewing angle to the operative field, and allowing the bowl to fall back, for safer reservoir placement.
